Below is the PADI Medical statement that all divers will be required to complete.
We recommend looking over this form prior to booking any diving trips or courses to see if you will answer yes to any of the questions. If you do then you will need a certificate from your doctor before you will be allowed to dive.
You can download the full form here to take to your doctor prior to coming to Koh Lanta. If you haven’t done this beforehand then we can take you to a doctor on the island, however this may delay your course and in the worst case you may not be allowed to dive.
